Practical information you should know

  • A list of your current medications and the date of your last flu or pneumonia vaccinations
  • Personal identification, including a photo ID and insurance card
  • Personal items that you will need during your stay

What NOT to bring

  • Valuable items or money should be left at home. St. Joseph’s Hospital is not responsible for lost or damaged items.

Smoking Policy

St. Joseph's Hospital campus and its affiliated medical buildings are tobacco-free and the use of tobacco products by employees, patients, medical staff, students, contracted personnel, auxiliary, volunteers, visitors, vendors and tenants is prohibited. This policy applies to all hospital owned and managed buildings, properties and grounds.


How do I get a copy of my medical records?

To obtain copies of your medical records, download and complete our Authorization Form to release your protected health information. Deliver the form to the Health Information Services (Medical Records) Department Monday - Friday between 8:00 a.m. and 4:00 p.m. Written authorization is required before production of the records. Allow a reasonable amount of time for processing your request. Be prepared to present state-issued identification to verify your identity.
